Low sodium diet is one of the best practices for nonphacomaco management of patients with heart failure. At present, it is found that there is still a need for the development of health services by involving families in order for patients to reduce the consumption of foods that are high in sodium. Inclusion criteria
Inclusion criteria: 1. Heart failure patient group who have severe levels of heart failure NYHA functional class I-III 2. can speak read and write Thai 3. have a smart phone for communication and can use the LINE application 4. Family members are willing to participate in the research
Exclusion criteria
Exclusion criteria: Heart failure 1. Schizophrenia, Bipolar disorder etc. 2. Age > 60 year has Mini-Cog score < 3 point 3. Sodium < 135 mmol/L
