Interventions

self and family management program is 8 weeks health education and skill practice in adult people with T2DM and their family members. The component of the program consists of 5 parts: 1) the SFMP eng
2) health education: to provide health education and skill training including knowledge about diabetes and complications, self-management tasks such as diet control, exercise, medications, and problem
3) family support: family involvement on diabetes SM tasks and how to assist the participant to perform tasks
4) motivational counseling: to encourage and motivate the participant to keep performing self-management behaviors by using telephone call and video call via line application
5) reinforcement: to motivate and follow-up the participant performing SM. Using MI approach and problem solving will be the main strategy in this study. ,Usual care is regular health care services pr

30 Years to 59 Years

Inclusion criteria

Inclusion criteria: 1 Diagnosed with type 2 DM by physician at the medical record 2 Age between 30-59 years old 3 Having HbA1C above 7% 4 Having the atherosclerotic cardiovascular disease (ASCVD) risk score equal or above 7.5% 5 Be able to perform activities of daily living by oneself 6 Be able to communicate in Thai language 7 Be able to contact via telephone 8 Committed significant family member involved into the program 9 Be willing to participate and give informed consent in the study

Exclusion criteria

Exclusion criteria: 1. Having a severe complications and other serious condition which made them unable to participate the program such as hospitalization, heart disease, volume overload, DKA etc. 2. Having insulin injection medication 3. Refusing to participate
