Inclusion criteria
Inclusion criteria: 1. Aged 18 to 85 years, patients older than 85 years have a high for adverse events such as arrhythmia and sudden cardiac arrest, and the patients who are 60 years and older will be assessed for cognitive impairment using the MINI COG test and score more than 3 points. 2. Being diagnosed with HFrEF with LVEF 15 to 40% on echocardiography by a physician because the patients who have LVEF less than 15% have a high risk for embolic stroke and cardiac events. 3. The functional status of heart failure patients classified by the NYHA Functional Classifications are class I-III 4. Present clinical stability for 24 hours after treatment optimization with no symptoms of dyspnea, pulmonary edema, and ankle swelling following readiness to CR program form. 5. Independently perform basic activities of daily living before admission 6. Physicians allow participation in the CR program and pass all criteria of heart failure discharge.
Exclusion criteria
Exclusion criteria: 1. Cardiac emergency 2. Underwent coronary artery bypass grafting (CABG) or valve surgery within 1 to 3 months. 3. Unable to perform exercise because of noncardiac causes, such as musculoskeletal system problems, and disabilities from exercise. 4. Hemodialysis or chronic kidney disease (CKD) stage 4 to 5 undergoing hemodialysis next 6 months. 5. Uncontrolled HT 6. Unable to perform completely 6 MWT at baseline before enrollment in the CR Program. 7. Have experience in CR programs or enroll in other exercise programs.
