Risk assessment, Bowel assessment every two hours
bed bath and perineum care,Nursing program with applying zinc oxide mixed with petroleum jelly for four days. ,Nursing program by applying zinc oxide followed by petroleum jelly for four days.

Inclusion criteria

Inclusion criteria: Older Patients 1. New cases of older patients aged 60 years who were admitted to the medical semi-ICU 2. Experiencing Incontinence 3. At high risk of IAD 4. At least four days admitted to the semi-ICU

Exclusion criteria

Exclusion criteria: Older patients 1. On the stage of palliative care 2. Already experienced IAD 3. Already experienced pressure injury or abscess 4. Having signs and symptoms of Allergic when applying zinc oxide and petroleum jelly
