Inclusion criteria

Inclusion criteria: 1 ) Patients with heart failure receiving services at the medical clinic and the heart failure clinic. Following inclusion criteria and exclusion criteria are established to select five patients. Inclusion criteria: A patient will be included in the study if he/she 1. is 18 years old and older, 2. is diagnosed with heart failure within the past 1 year, 3. is a patient with NYHA Functional Class II-IV, 4. is able to communicate in writing and listening to the Thai language, 5. has no cognitive impairment, measured with a Mini-cog score of > 3, 6. has a Barthel ADL index score of < 11, and 7. has a primary caregiver and can be contacted via smartphone. 2) The primary family caregivers. Of all selected five patients with heart failure, their five primary family caregivers also will be selected: one caregiver per patient. Following inclusion and exclusion criteria are specified to select these caregivers. Inclusion criteria: A primary family caregiver will be included in the study if he/she 1. is 18 years old or older, 2. is the primary family caregiver taking care of selected patients with heart failure in the past one year, and 3. is able to be contacted via smartphone.

Exclusion criteria

Exclusion criteria: A patient will be excluded from the study if he/she 1. has a history of psychiatric disease 2. has a moderate score anxiety PHQ9 score of greater than or equal to 13 points.
