Inclusion criteria
Inclusion criteria: 1. Patients who have a clinical diagnosis of type 2 diabetes mellitus with hypertension comorbidity 2. HbA1c values greater than 7 % 3. Systolic Blood pressure levels greater than 140 or diastolic blood pressure greater than 90 mmHg in the past six months 4. They are aged 40 to 59 years, both male and female 5. An inactive lifestyle (engage in less than 150 minutes per week of physical exercise by self-report) 6. Body Mass Index less than 30 kg per m2 7. Patients who have been prescribed at least one blood pressure medication (the nurses will verify medications) 8. Patients who have provided a signed informed consent form 9. Patients who have agreed to participate in the study for the full 12-week duration 10. Possession of a mobile phone able to receive data or messages.
Exclusion criteria
Exclusion criteria: 1. Blood pressure indicating the need for medication 2. Cardiovascular disease history of ischemic heart disease 3. Significant dementia or psychosis 4. Conditions limiting the ability to reduce blood pressure 5. The participants of intervention groups who did not exercise or record in the mobile application
