To compare the intubation rate, 28-day mortality rate, hospital length of stay, and ventilator free day between high-flow nasal and noninvasive ventilation in patient suspected sepsis with respiratory failure in emergency department. Inclusion criteria

Inclusion criteria: Patients who visit Ramathibodi ED with suspected sepsis and acute respiratory failure defined by at least one of these: respiratory rate > 25, SpO2 <90, PaO2-to-FiO2 ratio <300 or clinical presentation of respiratory distress (such as accessory muscles use or abdominal paradox)

Exclusion criteria

Exclusion criteria: 1. Patients needed for immediate intubation 2. Patients presented with cardiac arrest 3. Patients who referred from other department or hospital with high-flow nasal cannula or noninvasive ventilation 4. Patients with a do-not-intubate order 5. Patients on home positive airway pressure (PAP) therapy
