Eligibility
Inclusion criteria
Inclusion criteria: Inclusion criteria (Sample group) 1. Age 18 years and over. 2. Living family and caregiver is family members. 3. Have co-morbidities such as uncontrolled diabetes (HbA1C > 7%) and/or uncontrolled hypertension (systolic blood pressure > 140 mmHg and/or diastolic blood pressure > 90 mmHg). 4. Have a good level of consciousness, in person who are older than 60 years old have to pass screening for dementia using the 6-item cognitive function test, Thai version (Six Item Cognitive Impairment Test-Thai Version [6CIT]) with a score less than or equal to 7. 5. Able to read and communicate understandably in Thai. 6. Have smart phone that can install the LINE application. Inclusion criteria (Family members) 1. Age 18 years and over that member of family amd being a patient's caregiver including father, mother, husband, wife, children, grandchildren, sons-in-law, daughters-in-law. 2. Have complete awareness able to communicate. There are no limitations in communication, hearing, and vision. 3. Can participate in activities every time and willing to participate in research.
Exclusion criteria
Exclusion criteria: 1. The patient terminates participation in the activity. 2. Patients cannot participate in all activities every time. 3. There is a change in medication used for treatment, including blood pressure control medication and medicine to control blood sugar levels.
