Inclusion criteria
Inclusion criteria: 1. patients with diabetes and hypertension with CKD 3 complication 2. aged 40 to70 years, 3. blood pressure > 140/90 mmHg at least once in the last six months 4. HbA1C > 7% in the last six months 5. able to communicate, read, and write in Thai 6. lived with their family 7. no complications or severe illnesses during the study that might have caused any obstacle to activity participation.
Exclusion criteria
Exclusion criteria: 1. Need to be relocated away from Samutprakarn province during the study. 2. Request to be withdrawn from the study.
