Interventions
i) providing disease information and teaching behaviors to slow the progression of kidney disease to patients and family members via a two-hour session
(ii) teaching practical behavioral skills for appropriate diet and exercise
(iii) enhancing personal motivation via the Line application and motivation interviewing via telephone calls
(iv) enhancing social motivation with the support of a family member via the Line application
(v) employing technology to monitor behaviors and stimulate patient motivation
and (vi) assessing behaviors that were not practical, exploring barriers to behavior modification related to the repetition of knowledge and motivation, and teaching further practical behavior skills

40 Years to 70 Years
Inclusion criteria
Inclusion criteria: 1. patients with diabetes and hypertension with CKD 3 complication 2. aged 40 to70 years, 3. blood pressure > 140/90 mmHg at least once in the last six months 4. HbA1C > 7% in the last six months 5. able to communicate, read, and write in Thai 6. lived with their family 7. no complications or severe illnesses during the study that might have caused any obstacle to activity participation.
Exclusion criteria
Exclusion criteria: 1. Need to be relocated away from Samutprakarn province during the study. 2. Request to be withdrawn from the study.
