Inclusion criteria
Inclusion criteria: 1. Inpatient volunteer with chronic illness, age 10-19 years old 2. Plan to take two or more oral medications for at least 4 weeks 3. Clinically stable 4. Able to communicate Thai
Exclusion criteria
Exclusion criteria: 1. Already had medication chart
