Conditions

Critically ill patients who will be admitted to medical intensive care unit and have capillary blood glucose within 180- 400 mg/dl. basal bolus insulin, critically ill patient, hyperglycemia

Interventions

1. Discontinue oral or injecting antidiabetic drugs on admission. 2. Start total daily insulin dose 0.4 unitsper kg per day when the admission blood glucose concentration is more than 180 mg per dL 3

18 Years to 120 Years

Inclusion criteria

Inclusion criteria: 1. Adult age more than 18 years olds and 2. capillary blood glucose in between 180 - 400 mg/dl.

Exclusion criteria

Exclusion criteria: 1) Patients need intravenous infusion insulin therapy 2) Continuous enteral feeding 3) Patients in a shock state with vasopressor and nothing per oral 4) Patient with Raynaud phenomenon, which interferes with capillary blood glucose measurement 5) Hypoglycemia prior admission 6) Chronic liver disease (child-pugh score B and C) 7) History of insulin allergy or anaphylaxis 8) Surgical patients 9) Known pregnancy 10) Patient who cannot consent form or identify LAR within 48 hr
