Inclusion criteria

Inclusion criteria: Thai's patient age equally to or more than 18 years old. Admit to intensive or intermediate care in Ramathibodi hospital Intubated Consent to the trials.

Exclusion criteria

Exclusion criteria: During adjustment of ventilator needs On high ventilator setting (PEEP >12, FiO2 >0.6) Needs nebulization more frequent than 30 minutes Needs suction more frequent than 30 minutes Respiratory rate more than 20 times/minutes Pain with Numeric pain rating scale or visual analogue scale more than 6 Wish not to participate End of life care
