Inclusion criteria
Inclusion criteria: Those eligible for enrolling are type 2 diabetes patients aged 18 years or older, who have been diagnosed and followed up for type 2 diabetes for at least six months, have an HbA1c greater than 8% at the time of enrolment, and only one family member from each household will participate in the study. Furthermore, they should be able to understand Malay and have internet access at home.
Exclusion criteria
Exclusion criteria: Exclusion criteria include patients who are acutely ill, have psychiatric illness, who are bedridden and rely on nursing care to perform daily activities and who are intellectually disabled, as determined by a community-centred board.
