Inclusion criteria
Inclusion criteria: 1. Patients aged 18 years and over who have been diagnosed with heart failure stage C to D or NYHA functional class I to III and not at the stage of acute heart failure. 2. Patients with stable comorbid conditions are allowed to participate in this study which is researcher review from medical record. 3. Patients can communicate in Thai language because the questionnaire is in Thai language. 4. Willing to participate in the study.
Exclusion criteria
Exclusion criteria: 1. Patients with disabilities or abnormalities in thinking and understanding by reviewing the diagnosis history from the doctor in the medical record and assessment by researcher 2. Patients who are referred by the doctors to continue treatment at other hospitals or died during participation in this study 3. Patients who were unable to complete the questionnaire or requesting to withdraw while participating in the study
