Participants in the Pharmaceutical care using PROMPT questionnaires group (study group) received the patient-centered principle by assessing the patient&#039

s drug-related needs and finding problems with medication use (drug-related problems) of patients from the PROMPT questionnaire together with the medication use interview data, Patient medical records

s laboratory results and then evaluate the medication-related quality of life and health-related quality of life by PROMPT and KDQOL questionnaires, respectively. ,Participants in the routine hospital

Inclusion criteria

Inclusion criteria: 1. Age more than 18 years old 2. Able to communicate in Thai language 3. Consent to participate in the research

Exclusion criteria

Exclusion criteria: 1. Participants who were disabilities in thinking or understanding. 2. Participants who were referred or transferred to another hospital or death between trial. 3. Participants who were unable to complete the questionnaire, loss-follow up more than 2 weeks after a visit or requested withdrawal.
