This study will conduct aimed to improve family function, caregiver burden, and caregiver strain in family level and improve functional status, decrease depression, and prevent complications in individual level. STROKE / FAMILY BASED / NEUMAN SYSTEMS MODEL / FAMILY FUNCTION

Inclusion criteria
Inclusion criteria: Post stroke patients who meet the inclusion criteria as follows: 1) First episode ischemic stroke 2) Discharge from hospital within 1 months with moderate severe disability according to Modified Rankin Scale (MRS) 3) No complications . Family caregivers who meet the inclusion criteria as follows: 1) who were a main caregivers to take care post stroke patients and no salary gain 2) Living in the same house with the post stroke patient at least 1 months. Family members who meet the inclusion criteria as follows: 1) who have legal, biological, or emotional relationships with the post stroke patient, and provide assistance to family caregivers 2) Living in the same house with the post stroke patient at least 1 months.
Exclusion criteria
Exclusion criteria: Exclusion criteria for post stroke patients: On tracheostomy tube Exclusion criteria for family caregivers: 1) Change of family caregivers 2) Drop out or move out from house Exclusion criteria for family members: Drop out or move out from house
