Inclusion criteria
Inclusion criteria: phase I & III 1. live in 8 Hat Yai City Municipality-healthcare centers, Songkhla province, Thailand 2. score The Barthel Activity of Daily Living Index </= 8 3. need medical equipment phase II 1. healthy males who were firefighters or municipal officials 2. between 20 and 60 years old 3. no recent back pain.
