Inclusion criteria
Inclusion criteria: patientsFluently in Thai languag 1. 18 years or older 2. was diagnosed with HF 3. be able to contact by phone 4. fluently in Thai language Caregivers 1. 20 years or older 2. defined as a primary caregiver 3. Fluently in Thai language
Exclusion criteria
Exclusion criteria: 1. Presentation of any physical or mental disorder 2. life-threatening condition
