Interventions
The patients receiving pharmaceutical care with using the use of the Patient-
Reported Outcome Measure of Pharmaceutical Therapy Quality of life (PROMPT-
QoL). The PROMPT-
QoL can be suggested to be used as a screening tool for identifying DRNs,
and may provide a means to measure the humanistic outcome. The PROMPT-
QoL has 42 items within eight domains and one item about general attitude toward medication use.,The patients receiving pharmaceutical care (usual care)

Inclusion criteria
Inclusion criteria: 1) outpatients aged 18 or over 2) willing to participate in the study 3) having at least the following criteria (a)receiving medicine treatment from more than one physician (b)changing dosage regimens (c) taking at least five medicines (d) having abnormal laboratory caused by a medicine (e) not complying to treatment regimen (f) having an adverse drug reaction (ADR)
Exclusion criteria
Exclusion criteria: cognitive impairment or communicative problems
