Inclusion criteria
Inclusion criteria: An adult patient who is at least 18 years old and is admitted to medical ward
Exclusion criteria
Exclusion criteria: 1. A patient who receives anticoagulant therapy prior to the admission 2. A patient who is admitted for the investigation 3. A patient who is anticipated to be admitted to the hospital less than 72 hours 4. Pregnant women 5. A patient who declines to participate in the study
