Inclusion criteria

Inclusion criteria: 1. Age > 18 years old and Follow up at Warfarin clinic of division of cardiothoracic surgery of Siriraj hospital. 2. Been using warfarin at least 3 months and INR stable around 2.00-3.00 at least 2 times. 3. Patient ≥ 60 years old with TMSE score > 23 or dementia with a care giver. 4. Patient or a care giver or relatives are able to use the application and having all-day internet 5. Patient or a care giver or relatives are able to understand and communicate with Thai language 6. Patient regularly follow up at warfarin clinic

Exclusion criteria

Exclusion criteria: 1. Unable to sign the informed consent legally 2. Been diagnosed of any serious diseases, such as coronary artery disease without treatment, stroke without treatment, cancer, severe liver or renal disease
