Critically ill patient who was transferred out from ICU to general ward.

Interventions
Participants will receive transitional preparation program (teaching and training self-
care skill that related with active problem of patient) before transfer out ICU and follow up during stay at general ward 2 times (24 and 48 hr after transfer out ICU) by critical care nurse as a role
care skill) only at before transfer out ICU.,Participants will receive standard care before transfer out ICU.

18 Years to 65 Years
Inclusion criteria
Inclusion criteria: Inclusion criteria : 1)There are patients that stay in ICU at lest 72 hr. 2)Physician plan to transfer to general ward. 3)There are patients who have a next kin to continuous care. 4)There are patients who are anxiety that is assessed by Shortened Version of the SAI and no history of psychiatric. 5)Agree to participant in research and able to understand Thai language.
Exclusion criteria
Exclusion criteria: Exclusion criteria : 1)There are the patients who are critical ill again. 2)Who are need to withdrawn from research.
