Inclusion criteria
Inclusion criteria: Health providers working in the chronic care centres for Diabetes and Hypertension Adult patients diagnosed with Diabetes Mellitus and Hypertension Managers of the chronic care centres Patients who have had at least one prior visit to the particular hospital other than the current visit
Exclusion criteria
Exclusion criteria: Very sick patients Patients who do not consent First visit clients/Clients who have just been diagnosed on the particular visit
