Inclusion criteria

Inclusion criteria: All of the following: 1. patients aged 18 years old or more with dyspnea 2. able to well understand Thai language 3. need to be followed up at least once within 1-4 weeks later

Exclusion criteria

Exclusion criteria: One of the following: 1. unstable vital signs 2. unable to communicate
