Heart Failure Telehealth&#44

Interventions

A transitional telehealth program (TTP) provided comprehensive discharge planning and home health monitoring through a mobile phone&#45
based application and telephone calls following discharge from the hospital for six weeks plus usual care ,Usual care provided health education before being discharged&#44
telephone contact during the hours of 8:30 am to 4:30 pm on weekdays for a consultation about health problems and provided Heart failure booklet.

Eligibility

Age
18 Years to 100 Years

Inclusion criteria

Inclusion criteria: 1. Adult patients, diagnosed with HF within the last 12 months. 2. Ability to be accessed by mobile phone and internet signals. 3. Ability to use the mobile app either with or without assistance from a caregiver. 4. Having a Thai Mental State Examination (TMSE) score >23

Exclusion criteria

Exclusion criteria: 1. Having cancer, chronic kidney disease, or surgery planned within two months, and/or were on dialysis. 2. Submitting self-monitoring data fewer than 30 days of the total 37 days.
