Inclusion criteria
Inclusion criteria: 1) Agd atleast 60 years 2) Patients with hypertension and/or type 2 diabetes 3) Need Non medical care 4) Live in Mueg district, Chiangmai province
Exclusion criteria
Exclusion criteria: 1) Bedridden patients 2) End-stage renal disease with Renal replacement therapy, or eGFR <30 ml/min/1.73 m2 3) History of Myocardial infraction within 6 months ago. 4) Heart Failure: New York Heart Association grade 3 (NYHA) 5) Participant with other clinical trials
