Inclusion criteria

Inclusion criteria: aged 18 or older with mechanical ventilation more than 48 hours&#44; planed extubation and passed spontaneous breathing trial (SBT) and cough leak test&#44; with ineffective cough measured by cough peak flow < 270 LPM.

Exclusion criteria

Exclusion criteria: pneumothorax&#44; hemodynamic instability&#44; hemoptysis&#44; unfollow to command&#44; gastrointestinal hemorrhage&#44; bronchospasm&#44; upper airway obstruction
