Inclusion criteria

Inclusion criteria: The inclusion criteria were as follows: the potential participants had a diagnosis of HF (New York Heart Association (NYHA) class II to III) by a physician, were aged above 20 years old, were able to communicate in Mandarin or Taiwanese, and could perform activities of daily living independently

Exclusion criteria

Exclusion criteria: The exclusion criteria were a diagnosis of dementia or a psychiatric disorder, inability to be contacted by phone or refusal of follow-up
