60 Years to 100 Years

Inclusion criteria

Inclusion criteria: 1. Type 2 Diabetes Milletus patients who are currently followed up at Continuity of Care clinic or Out-patient Clinic of Department of Medicine, Faculty of Medicine Siriraj Hospital. 2. Patients or their caregivers who have an account and are able to use Line application. 3. Patients who have HbA1c at recruitment period equal to or greater than 7.5%. 4. Patients or their caregiver who agree to participate in the study and sign informed consent.

Exclusion criteria

Exclusion criteria: 1. Those who have language barrier, cannot read or write well in Thai. 2. Those who have severe vision or hearing problems.
