Inclusion criteria

Inclusion criteria: Hospitalized medical patients admitted within 48 hours Malnutrition assess by Nutrition Alert Form (NAF) class B or C

Exclusion criteria

Exclusion criteria: Terminally ill patients with an end-of-life care plan Patients who are already consulted with the clinical nutrition team. Unable to obtain bodyweight or height Body mass index >= 30 kg/m2 Patients who are admitted for planned procedure
