Inclusion criteria

Inclusion criteria: Thai patients over 18 years of age whom LV function less than 40% by echocardiogram, MRI, LV ventriculogram or thallium scan within 1 year which the followings condition 1. Present of epicardial coronary stenosis more than 75% in left main or proximal left anterior descending artery (LAD) by coronary angiogram and/or present of two major epicardial coronary stenosis. 2. Prior history of myocardial infarction (MI) 3.Prior history of revascularization by coronary artery bypass graft (CABG) or percutaneous coronary intervention (PCI) 4. MRI pattern compatible with ischemic cardiomyopathy

Exclusion criteria

Exclusion criteria: 1. Patient with a history of myocardial infarction within 30 days when access LV function 2. Patient with a history of acute coronary syndrome within 30 days when access LV function 3. Patient with a history of significant valvular stenosis or regurgitation that may explained LV dysfunction 4. Patient whom enrolled in blinded clinical trials 5. Patients whom life expectancy less than 1 years 6. Patient who lost to follow up 7. Patient who refuse to participate the registry 8. Patient with unstable hemodynamic
