Inclusion criteria
Inclusion criteria: An extensive data collection will be performed as a retrospective review in PowerChart. Inclusion data will include surgical consult placed by any other service (including but not limited to emergency medicine, internal medicine) with elevation of lactic acid at the time of consult for the time period January 2009 through December 2013. Data points collected will include, but may not be limited to: patient age, patient gender, presence of comorbidities, reason for consult, whether patient was taking specific medications (metformin, isoniazid, nucleoside reverse transcriptase inhibitors) as an outpatient, initial and subsequent lactic acid levels, initial and maximum glucose levels on admission, presence of ketones on urinalysis, white blood cell count, creatinine, hemoglobin, potassium, vital signs on presentation and immediately prior to surgical consultation, which cultures were drawn (including but not limited to sputum, blood, urine), results of such cultures, subjective review of systems at time of consult (including but not limited to nausea, vomiting, abdominal pain, diarrhea, bloody bowel movements), physical exam at time of surgical consult, fecal occult blood test results, all radiologic imaging performed and subsequent results, day of hospital course that surgical consultation was obtained, number of hours after lactic acid level was drawn that surgical consultation was obtained, whether or not operative intervention was performed, operation performed, anatomic pathology of any removed specimen. In addition, a chart review and subjective "reason for consultation" will be recorded. Using Excel, a spreadsheet will be made to review all quantitative and qualitative data points that we wish to study. Using statistical analysis we will analyze all gathered qualitative and quantitative data to evaluate for potential factors that seem to predict whether operative intervention will be necessary following a consult for general surgery involving lactic acidosis.
Exclusion criteria
Exclusion criteria: Any disease state which could falsely elevate lactic acid including diabetes, chronic kidney disease, metformin use, and liver disease
