Interventions

We developed this program for this study. Participants underwent a 10&#45
week goal attainment nursing program to prevent cardiovascular diseases in hypertensive patients. This program emphasized goal attainment and promoting self&#45
management behavior. The initial process begins with interactions to identify obstacles and common health problems and inform CVD risk factors. The program then creates goals for the patients. These g
recording&#44
and evaluation of goals. The difficulties and physical outcomes were tracked and supported for 10 weeks via a telephone call. At the last session&#44
patients received feedback on changeable behaviors and evaluation on attainment of goals in a discussion group. All of these processes improve on the continued interaction between nurse and patients.

Inclusion criteria

Inclusion criteria: 1.diagnosis of hypertension at least six months prior to the study 2.SBP over 130 mmHg and/or DBP over 85 mmHg 3.prescribed at least one antihypertensive drug 4.has at least one additional cardiovascular risk factors: a) a history of CVD in family b) smoking c) diagnosed with DM d) hyperlipidemia (any of: total cholesterol > 200 mg/dL, triglyceride > 150 mg/dL, low-density lipoprotein > 130 mg/dL, high-density lipoprotein 25 kg/m2 or waist circumference 90 cm in male, > 80 cm in female). 5. own a mobile phone while being conversant and literate in the Thai language.

Exclusion criteria

Exclusion criteria: 1.patients with pre-existing cardiovascular complications 2.patients who are sent to receive special treatment and will not return to the primary care unit
