Inclusion criteria

Inclusion criteria: All inpatient (IPD) patients with diagnosis of heart failure confirmed by both primary physician and 2 major or 1 major and 2 minor Framingham criteria were included after informed consent

Exclusion criteria

Exclusion criteria: 1) Pregnancy patients 2) Confused patients or disabled patients that cannot made their own decision 3) Patients that cannot be reached by phone call during follow up
