This telehealth program was modified and developed from telehealth program by O’Connor and team
and a study by Kleinpell et al (2015). Using Transitional care model as the framework,
the researcher and her advisor developed the telehealth program which intended to reduce readmission and increase functional status. The program provided information through smart phones which include

18 Years to 90 Years
Inclusion criteria
Inclusion criteria: Patient has to meet the following inclusion criteria: 1) underwent CABG for the first time ; 2). have a New York Health Association functional classification of I–III; 3). Thai Mental State Examination (TMSE) score >23; 4). Patients can communicate and understand Thai language; 5). Patients can communicate via a smartphone.
Exclusion criteria
Exclusion criteria: Disability or limitation in walking
