On warfarin Anticoagulant Discharge Telemedicine Telephone follow&#45

Interventions

Following discharge of the participants in the telephone follow&#45
up group&#44
pharmacists made one telephone call before the next follow&#45
up on the following day based on their scheduled visit: day 3 for a 1 week scheduled visit&#44
day 7 for a 2 weeks scheduled visit&#44
and day 14 for a 3 or 4 week scheduled visit. For the second and subsequent visits within the 3&#45
month study period pharmacists continued to make telephone calls on a regular interval as follow: day 3 for a 1 week scheduled visit
days 3 and 7 for a 2 weeks scheduled visit&#44
days 3&#44
and 14 for a 3 weeks scheduled visit
and days 7&#44
and 21 for a 4 weeks scheduled visit. During each 10&#45
25 minute telephone call&#44
pharmacists performed medicine use review by enquiring patients about problems/obstacles in undertaking warfarin including adverse events and complications&#44
assessing medication adherence&#44
and giving reminders for the next scheduled visits. Pharmacists promptly addressed any problems identified during the telephone call with patients.,standard pharmacy service for patients discharged wi

Inclusion criteria

Inclusion criteria: Patients aged 20 years or older were eligible for inclusion if they were (i) prescribed with warfarin upon discharge, (ii) had no schedule for surgery within three months after discharge, (iii) were able to manage their own medication, (iv) could communicate in Thai, and (v) possess a telephone.

Exclusion criteria

Exclusion criteria: Those who already took warfarin before admission and had INR values in target ranges throughout the past three months were excluded.
