Interventions
The samples in the control group in the trial will receive routine care on the first day of registration on the CABG waiting list from the nursing team and surgeon at the Cardiothoracic Outpatient Cli
Siriraj Hospital,
Bangkok. The participants will be offered verbal and written advice with a description of the operation,
preparation for upcoming CABG surgery,
post CABG care and self-
care while waiting for CABG surgery at home. One heart handbook containing information on preparation for admission and recovery information will be distributed to each patient. During the waiting tim
most CAD patients will not have any appointments with the surgeon since they will already have been placed on the waiting list.,C-
ups for 6 weeks.During initial meetings with participants,
the nurse-
researcher will perform the first task of screening by assessment and identifying the patients’ health problems. Next,
the nurse will collaborate with the participants,
caregivers and healthcare team to identify,
and develop an individualized care plan and goals. The nurse will implement the plan as follows: 1) educating the participants and caregivers to have essential knowledge of care in order to maintain
2) monitoring and responding quickly to worsening symptoms and 3) providing psychological support for the participants. In addition,
researcher will review the medication plan,
instruct and remind the participants and caregivers about each prescribed medication,
including information on rational use,

Inclusion criteria
Inclusion criteria: The patients eligible for participation in the study if they are: 1) aged 18 years old and over; 2) new cases of registration on the CABG waiting list; 3) patients with no history of previous cardiothoracic surgery; 4) able to read and understand the Thai language and 5) able to be contacted by telephone.
Exclusion criteria
Exclusion criteria: Exclusion criteria The patients will be excluded from the study if they: 1) have a Depression Subscale (HADS-D) of the Thai version of Hospital Anxiety and Depression Scale (Thai HADS) score of more than 15 points, thereby indicating severe symptoms for depression ;2) have a significant life-threatening non-cardiovascular disease such as advanced cancer; 3) have a history of psychiatric disorders or cognitive dysfunction such as neurologic diseases or dementia with a Thai Mental State Examination (TMSE) score of less than 24 points, thereby indicating cognitive impairment; and 4) have a history of functional inability such as immobilization syndrome.
