Inclusion criteria

Inclusion criteria: The household informant will be selected from household members available during the survey based on the following inclusion criteria: -Age > 18 years -Literate in Thai -Resident of Si Kao district If there is more than one eligible subject, we will choose the one who is more influential in household decision making. However, all household members may help completing the questionnaire.
