Age
20 Years to 80 Years

Inclusion criteria

Inclusion criteria: 1. on mechanical ventilation > 72 hrs 2. age > 20 yers 3. start to weaning MV with CPAP or SIMV 4. Glasgow coma scale < 8

Exclusion criteria

Exclusion criteria: 1. unstable fracture of upper extremities 2. unstable vital sign (HR >120 mmHg, RR> 30 breathe/min, SpO2 <90%, SBP >140 mmHg or <90 / mmHg) 3. patients have restricive lung disease 4. APACHE II score <11 or > 20 5. not upright 4. limit shoulder ROM
