Chronic diabetic wounds

Interventions

Participants will be recruited from in-ward patients as well as patients attending the wound clinics of the Colombo South Teaching Hospital. Consenting participants will be randomized into three arms.

Eligibility

Inclusion criteria

Inclusion criteria: 1. Age over 18 years 2. Chronic diabetic wounds which have lasted over 1 month 3. No clinical evidence of occlusive arterial disease

Exclusion criteria

Exclusion criteria: 1. Uncontrolled diabetes needing inward management 2. Clinical evidence of concurrent occlusive arterial disease (OAD). 3. Other co-morbidities requiring use of systemic antibiotics 4. Any patient needing surgical intervention or use of other methods (rotational antiseptics) to control wound infection. I 5. Known hypersensitivity to acetic acid or ascorbic acid

Design outcomes

Primary

MeasureTime frame
Wound healing according to PEDIS score (Perfusion, Extent/size, Depth/tissue loss, Infection, and Sensation) and an objective wound assessment questionnaire [At baseline, at the end of one week and at the end of 1 month]

Secondary

MeasureTime frame
1. Bacterial colony count of the wound 2. pH of the wound [Both secondary outcomes will be assessed at the following time points. Day 1: Pre swab (prior to antiseptic instillation), 10 minutes after instillation, 1 hour after instillation, 4 hours after instillation Day 3: Pre swab (prior to antiseptic instillation), 10 minutes after instillation, 1 hour after instillation, 4 hours after instillation At the end of one month ]
