18 Years to 50 Years

Inclusion criteria

Inclusion criteria: Individuals aged between 18 and 50 years; transgender persons classified according to gender identity as transgender men or transgender women; in the initial phase of gender-affirming hormone therapy according to the standardized clinical protocol of the Brazilian Public Health System; transgender men receiving testosterone; transgender women receiving estrogen, either alone or in combination with cyproterone or spironolactone, according to individual clinical indication

Exclusion criteria

Exclusion criteria: Refusal to sign the informed consent form; cognitive impairment preventing adequate understanding of the study procedures; use of analgesics, nonsteroidal anti-inflammatory drugs, or opioids within 24 hours prior to assessments; history of craniofacial trauma; previous facial plastic surgery; history of facial aesthetic procedures; active orthodontic treatment; extensive oral rehabilitation with potential impact on facial morphology; pregnancy; presence of pacemakers, implantable defibrillators, or other internal electronic devices

MeasureTime frame
To evaluate changes in total facial volume (cm³) during the first year of gender-affirming hormone therapy. Differences in facial morphology over time are expected, with the main comparison between baseline and 12 months. Outcome assessment will be performed using three-dimensional structured-light facial scanning, with digital model generation and volumetric difference calculations across time points (baseline, 3, 6, and 12 months).

To evaluate regional facial, skeletal, and body morphological changes during the first year of gender-affirming hormone therapy. Changes in the volumetric distribution of facial soft tissues (mandible, maxilla, chin, forehead, and lips), maxillary and mandibular bone structures, and body composition parameters over time are expected, as well as differences in the magnitude and temporal pattern of these changes between transgender men and transgender women. Outcomes will be assessed using three-dimensional structured-light facial scanning for regional soft tissue volumetric analysis (baseline, 3, 6, and 12 months), cone-beam computed tomography for volumetric evaluation of maxillary and mandibular bone structures (baseline and 12 months), and segmental multifrequency bioelectrical impedance analysis for body composition assessment (baseline, 3, 6, and 12 months), including total body water, intracellular and extracellular water, skeletal muscle mass, lean mass, body fat mass, body fat percentage, body mass index, and basal metabolic rate.
