Eligibility

Age
18 Years to 90 Years

Inclusion criteria

Inclusion criteria: Patients with heart failure; who were hospitalized and / or searching the emergence in the last three months due to decompensation by the disease; in clinical follow-up in heart failure clinic of the University Hospital Pedro Ernesto; both sexes; older age or equal to 18 years; not present any functional problem, which prevents him from speaking, writing or interfere with cognition; have telephone contact and availability monitoring.

Exclusion criteria

Exclusion criteria: Heart failure patients in very advanced stage with no prospect of life (greater than or equal to 6 months); and / or with other comorbidities in terminal stage that might interfere with evaluation; Stage IV according to the functional classification of the New York Heart Association.
