Eligibility

Age
30 Years to 80 Years

Inclusion criteria

Inclusion criteria: Patients who are diagnosed with Diabetes mellitus type 2; both sexes; age between 30 and 80 years; with access to fixed or mobile phone line.

Exclusion criteria

Exclusion criteria: Patients who are diagnosed with chronic complications such as diabetic retinopathy; diabetic neuropathy; diabetic nephropathy and amputation. Patients who do not attend the call for more than five consecutive attempts.

Design outcomes

Primary

MeasureTime frame
An improvement of metabolic control in the period of 18 months which is verified through examination of glycohemoglobin collection by finding of a 1,0% variation in the measurements before and after intervention.

Secondary

MeasureTime frame
An increase of self-care practices within 18 months which is verified by applying the questionnaire of self-care in Diabetes by finding at least a score of 5 points in the measurements before and after intervention; Increased self-eficcacy within 18 months which is verified by applying the range of psychosocial autoeficacia on Diabetes by finding at least a 3.8-point score in the measurements before and after intervention; An improvement of psychological attitudes in the period of 18 months, which is verified by applying the questionnaire of attitudes on Diabetes by finding at least a score of 70 points in the measurements before and after intervention.
