Inclusion criteria

Inclusion criteria: Individuals aged 60 years or older. Of both sexes. Registered at the primary health care unit where the study will be conducted. Classified as at risk of frailty according to the Clinical-Functional Vulnerability Index-20 (IVCF-20), with a score between 7 and 14. Able to understand and respond to study procedures. Willing to participate and who provide written informed consent

Exclusion criteria

Exclusion criteria: Individuals with severe cognitive impairment that prevents communication or understanding of the study guidelines. Individuals who do not attend both nursing consultations and at least six group meetings during the intervention period

Design outcomes

Primary

MeasureTime frame
A change in clinical-functional vulnerability among older adults is expected. The outcome will be assessed using the Clinical-Functional Vulnerability Index (IVCF-20), applied one week before the start of the intervention and 30 days after its completion. Differences in total IVCF-20 scores between the two time points will be analyzed, as well as changes in participant classification (robust, at risk of frailty, and frail), considering score reduction as indicative of improvement

Secondary

MeasureTime frame
A change in the level of self-care among older adults is expected. The outcome will be assessed using the Appraisal of Self-Care Agency Scale – Revised (EAAA-R), applied before the intervention and 30 days after its completion. Variations in total scores will be analyzed, considering higher scores as indicative of improved self-care capacity;Changes in health-related behaviors are expected. The outcome will be assessed using a structured questionnaire on lifestyle, including physical activity, diet, alcohol consumption, and smoking, applied before the intervention and 30 days after. Frequencies and proportions of the variables will be analyzed, considering improvements in habits as a positive outcome;Changes in anthropometric parameters are expected. The outcome will be assessed through measurements such as body weight, body mass index, waist circumference, and waist-to-hip ratio, collected before the intervention and 30 days after. Differences between measurements at the two time points will be analyzed;Improvement in functional capacity is expected. The outcome will be assessed using variables related to activities of daily living included in the IVCF-20, evaluated before and after the intervention. Changes in responses related to functional independence will be analyzed
