Eligibility

Inclusion criteria

Inclusion criteria: Eighteen documents of physician handover session before intervention and 18 after the intervention randomly selected for monitoring.

Exclusion criteria

Exclusion criteria: Clinical documents of handover session which physicians do not agree with the voluntary participation in the study.

MeasureTime frame
The quality of information registered in the medical handover recording; using a tool with following criteria: patient identification, hospital location, clinical history, facts about the hospitalization, diagnosis, alerts (severe conditions, critical problems or values, adverse events, allergies), plan care.;Security of information recorded on the medical handover; using the following criteria: identification of document editors (authenticity and certification), access profile, and traceability.

MeasureTime frame
Lenght of the transfer of care sessions between doctors shifts; evaluated by an external observer using a chronometer.
