Design outcomes

Primary

MeasureTime frame
To evaluate the effects of case management associated with a program for falls prevention in community-dwelling fallen elderly. Using the case management method, we will observe through an initial assessment, containing sociodemographic data such as age, sex, color, marital status, years of schooling, living alone, income and general health such as use of medications, presence of morbidities, history of falls, hospitalization, level of physical activity, functional activities and risk for fracture, assessment of motor measures, neuropsychological measures, safety of the environment, foot and shoe, the risk and education on falls. The intervention will take place for 16 weeks and then a reassessment will be done after 16 weeks, after 6 weeks the same assessment will be done. Also, at the end of the intervention, the participants will answer two questionnaires, one about satisfaction with the intervention and another about adherence to the intervention. The occurrence of falls will be collected from the beginning to 1 year of follow-up by means of a falls diary. The health economic evaluation will be performed after 1 year of follow-up. Falls will be tracked using a falls calendar that will be given to the volunteer at the beginning of the intervention. The volunteer will be asked to fill out the calendar from baseline to 1 year of follow-up.
