18 Years to No maximum

Inclusion criteria

Inclusion criteria: Be registered in the Family Health Units (USF) included in the study with a diagnosis of Type 2 Diabetes Mellitus; of both sexes; older than 18 years

Exclusion criteria

Exclusion criteria: Have a health problem that makes verbal communication difficult/impossible and/or traveling to the Family Health Units; not have telephone contact; change of coverage area of ??the Family Health Units during data collection

MeasureTime frame
It is expected that through the following proposal that the intervention can contribute to clinical stability measured through laboratory tests: glycated hemoglobin, fasting blood glucose, total cholesterol and fractions, urea and creatinine;It is expected that through the following proposal that the intervention can contribute to behavioral change, which will be assessed through two questionnaires, namely: Diabetes Self-Care Questionnaire and Diabetes Management Self-efficacy Scale
