Inclusion criteria
Inclusion criteria: > 18 years old; telephone number avaiable for after discharge; admitted with heart failure in emergency;
Exclusion criteria
Exclusion criteria: < 18 years old; cardiac surgeryc in the last 3 months;

| Heart Failure treatment adherence, determined through questionnaires adherence applied at the end of the study in control anda intervention groups.;Greater adherence to the Heart Failure treatment by the intervention group, determined through adherence questionnaires, will be applied at the end of the study, in the comparative with control group.;Greater adherence to the Heart Failure treatment by the intervention group with 25,3%, determined through adherence questionnaires, was applied at the end of the study, in the comparative with control group with 7,6%.;Lower rate of rehospitalization in 90-day period in the intervention group, will be analyzed by comparative telephone call, compared to the control group.;Lower rate of rehospitalization in 90-day period in the intervention group with apresent 26% of rehodspitalization, will be analyzed by comparative telephone call, compared to the control group with apresent 39% of rehospitalization. | — |
