Age
18 Years to No maximum

Inclusion criteria

Inclusion criteria: Patients discharged from the hospital within the study period, regardless of gender; who are at least 18 years old will be included in the study

Exclusion criteria

Exclusion criteria: Patients transferred to other hospital units or healthcare institutions before discharge; patients who do not respond to the Hospital Consumer Assessment of Healthcare Providers and Systems instrument after hospital discharge will be excluded

It is expected that bedside hospital discharge performed by the nurse, guided by a standardized instrument, can favor the transition of care, offering more safety and quality after hospital discharge
