Inclusion criteria
Inclusion criteria: People aged 60 years or over; both sexes; registered and monitored by the Family Health Strategy - ESF teams; classified in relation to the degree of clinical-functional vulnerability
Exclusion criteria
Exclusion criteria: Long-term stay (equal to three months or more) in a hospital, institution or nursing home; record of receiving an alternative type of integrated care
